The Sicilian lemon is kind of picky about temperature - if you live somewhere that gets pretty cold in winter, it can go months without flowering at all, and then it really does look like that every-other-year thing. People tend to ignore that it needs a temperature contrast to trigger flowering, it's not just about light and water.
It fruits year-round, especially if it gets plenty of light and heat! But if yours is in a place with harsh winters or you're not taking great care of it, fruiting can be more irregular, of course.
That division into "every year" or "every other year" is pretty oversimplified, because the Sicilian lemon loves to fruit all the time, but the intensity changes a lot depending on the season and the care you give it - under poor conditions it really can have some long gaps.
