The most important thing is to coordinate with trustworthy people who won't blab. Use a WhatsApp group or private chat to organize the details, and if possible, have someone close to the birthday person ask him to go somewhere without raising suspicion - like "let's go to the movies" or whatever excuse works. Keep him from seeing weird purchases, decorations around the house, or suspicious messages, so maybe it's better to store everything somewhere else until the last minute and have people gossip to him as little as possible.
Do you already have the venue confirmed? What almost nobody mentions is that someone has to keep the birthday person distracted on the actual day - you can't just wing it because he'll notice the weird vibe in the group. The private chat idea was already mentioned, but I'd add that you should designate just one person as "project manager" to prevent five people from sending contradictory messages and then someone accidentally letting something slip.
