Don't just throw it loose in your backpack with other stuff because cables and keys will scratch it without you even thinking about it. Best bet is a decent case plus a padded pouch or a separate little bag inside the backpack, so you keep the phone away from everything else. If the backpack drops or takes a hit, that padding saves you from a heart attack and probably a broken screen too.
I agree that a case is necessary, but honestly it depends on how much you care about your phone and how accident-prone you are. If your backpack is one of those that you bang against everything, a thick case plus a padded sleeve will save your iPhone's life, but if your backpack is more careful and organized, a regular case and just not putting keys or sharp things in the same compartment will keep it safe.
