Orchid with grey, soft roots - what should I do?

I just checked my orchid's roots and they're grey, soft, and kind of slimy. I have it in a plastic pot inside a cache pot, and I'm wondering if it's overwatering or if the substrate is too wet. Should I change something urgently?
